Quick answer: The strongest option for a San Antonio med spa depends on one question most clinics never ask: who is legally responsible for the before and after photos on your website. Studio 3 Marketing out of Austin has the deepest aesthetic portfolio, Funnel Boost Media is the most credentialed San Antonio agency, and ByteSimple is the only company on this list that publishes a Business Associate Agreement and a photo consent workflow as part of the build. That last detail matters more than design taste. How we built this list, and what we deliberately ignored

Three filters produced the eight companies below.

The company had to show actual aesthetic or medical work. Not a healthcare page on a general agency site. A named clinic, a portfolio piece, or a case study we could look at. Med spa websites have specific jobs that a restaurant site does not: a treatment menu that reads clearly to someone who has never had the procedure, a results gallery, provider credentials, and a booking path that connects to whatever practice software the clinic already runs.

The company had to be reachable for San Antonio work. That means a local operation or a specialist that takes Texas clients. Both are valid, and the tradeoff between them is a real part of this decision.

We looked at what each company says about compliance, and we report it as found. Some publish specifics. Most say nothing. Silence is not proof of a problem, but in this category it is worth knowing before the first call, because the question lands on the clinic and not on the agency.

We ignored directory rankings that agencies can pay to appear in, and we ignored design awards, because a beautiful site that does not convert or that creates regulatory exposure is not a good outcome for a clinic.

We pulled our own Keyword Planner data for this vertical rather than repeating industry claims. The set contains 222 unique keywords across national, Texas and San Antonio DMA geographies, with 576 rows carrying at least ten monthly searches.

Inside the San Antonio DMA, the head term med spa san antonio and its variants carry roughly 1,300 searches per month, with a top of page bid range of only $1.87 to $6.80. Lip injections san antonio carries 720 searches per month at $1.98 to $6.12. Read those numbers next to each other, because they describe a market that behaves nothing like the other categories we have covered here.

In legal, the constraint is cost. A single top of page click on an accident attorney term in this metro can run past $900, so the entire argument is about not paying for the click. In med spas the opposite is true. The demand is large and the clicks are inexpensive, at least for the core discovery terms. Traffic is not the scarce resource here.

What is scarce is conversion and trust. Dozens of clinics in Bexar County compete for those same 1,300 monthly searches, most of them running the same treatment menu, and the visitor decides between them on the website in under a minute. That makes the site itself the bottleneck, which is exactly why website design is worth doing well in this vertical and why the rest of this page focuses on what the site has to accomplish rather than on how to buy traffic.

The laser hair removal spread is worth one note. When a treatment carries a bid reaching $76.42, that tells you the clinics bidding on it are confident in the lifetime value of the patient behind it. High value treatments deserve their own dedicated page rather than a line item on a treatment menu, and most med spa sites in this market do not have them.

This is the section we have not found a single competitor in this market address directly, and it is the most consequential thing on the page.

The before and after gallery is the highest converting element on a med spa website. It is also the riskiest.

Patient photos are protected health information when the individual can be identified, and identification is a lower bar than most clinics assume. Blurring a face or covering the eyes does not automatically make an image anonymous, because tattoos, birthmarks, jewelry, scars, hairstyles and even the background of the room can still identify a person.

Using those photos in marketing requires a specific written HIPAA authorization that is separate from the general treatment consent the patient signed at intake. Published guidance on this is consistent and unambiguous: a general line saying information may be used for marketing purposes is not sufficient. The authorization needs to describe what was photographed and name the specific uses being authorized, meaning the website, Instagram, paid ads and print are separate permissions rather than one blanket yes. Signed authorizations have to be retained, and the retention expectation for HIPAA compliance documentation is at least six years. The same logic applies to written patient testimonials.

Now connect that to hiring a web designer. The vendor who receives your photo library, uploads it, organizes it and hosts it is handling PHI on your behalf. That relationship is what a Business Associate Agreement exists to govern. Publishing a gallery without proper authorization is among the most commonly cited violations in this category, and operating without a BAA in place with a vendor who touches patient data is another.

The practical question for a clinic owner is short and you can ask it on the first call. Will you sign a Business Associate Agreement, and what is your process when we hand you a folder of patient photos. A vendor with a real answer will describe a consent workflow. A vendor without one will tell you the photos look great.

Of the eight companies on this list, exactly one publishes both a BAA and a photo consent workflow as part of the build. Two others publish HIPAA practices at the agency level. The rest say nothing publicly, which is not evidence of a problem but is a reason to ask.

The second question: who the site says owns the clinic

Texas prohibits non physicians from owning a medical practice or employing physicians, a doctrine usually called the corporate practice of medicine. A med spa crosses into the practice of medicine the moment it offers injectables, laser treatments or peels beyond cosmetic scope, which describes essentially every med spa. That means physician ownership or a compliant structure such as a management services organization, and it means a physician, physician assistant or advanced practice registered nurse must be on site or immediately available for an emergency consult when those procedures are delegated. Esthetician ownership of a medical aesthetics practice is not permitted, and an esthetician license does not authorize medical acts regardless of how a device is marketed.

The Texas Medical Board has issued guidance to physicians about med spas and similar businesses that rely on delegation and supervision, reporting that it continues to encounter businesses with no physician involvement or inadequate physician involvement.

Where this touches a website is in what the site represents. A site that foregrounds the aesthetician and never shows the supervising physician, or that describes the business in a way inconsistent with its actual structure, is a public document about a regulated arrangement. Provider credential transparency is also one of the things visitors evaluate, so this is a conversion issue and a compliance issue at the same time rather than a tradeoff between them.

Bring your structure to whoever builds the site, and have your own counsel confirm the site describes it accurately. No web vendor on this list, including us, can make that determination for you.

1. Studio 3 Marketing

Studio 3 Marketing is a medical and aesthetic marketing agency with its national headquarters in Austin, plus offices in Los Angeles and Tampa, operating for more than fourteen years.

What it is genuinely good at. Portfolio depth in exactly this vertical, which is uncommon. Named aesthetic clients include Aurora Medical Spa, Ciaravino Total Beauty, Genesis Lifestyle Medicine, Portland Plastic Surgery Group and Athenix, with published results including 199 percent traffic growth in six months for Aurora and multi year compounding growth for Genesis. The service range covers branding, website design, video and photography in house, SEO and AI search optimization, digital advertising and lead management, with a stated team of more than forty marketing professionals. In house photography matters more here than in most verticals, because a med spa site lives on imagery and stock photos of other people's faces undercut the entire trust argument.

Where to look closely. This is an agency built for clinics with real marketing budget and multi location ambitions, and it does not publish pricing. It is in Austin rather than San Antonio, which for design work is mostly irrelevant but matters if you want someone who can walk into the clinic to shoot. Funnel Boost Media is the most credentialed agency headquartered in San Antonio itself, founded in 2012 by Ryan Duncan and veteran owned.

What it is genuinely good at. Demonstrated search capability rather than claimed capability. Its own domain carries a domain authority of 34 with more than 2,600 referring domains, it ranks organically for the competitive terms it sells, and it has been named to a Forbes list of America's best digital marketing companies. Healthcare is one of its core verticals alongside home services, legal and ecommerce. It also maintains dedicated San Antonio service pages for answer engine and generative engine optimization, so a clinic that wants the website and future AI visibility from the same vendor can get both.

Where to look closely. Med spas are a subset of a healthcare vertical that is itself one of several, so ask how many aesthetic clinics they currently run and ask to see two of those sites. Ask about minimum commitment. Ask the gallery and BAA question, because we did not find published specifics.

Best fit. A clinic that wants the strongest local track record and is comfortable being one account inside a large agency.

3. Get Local Presence

We build and then keep operating the site as one ongoing monthly plan, rather than selling a project that ends at launch.

What we are genuinely built for. Med spa sites decay faster than most because the treatment menu changes, pricing changes, providers change and the gallery grows continuously. A project based build is correct on launch day and drifts from then on, and the drift is expensive because a treatment page that no longer matches what the clinic offers costs bookings quietly. ByteSimple is an owner operated San Antonio web studio run by Danny Tsui, serving San Antonio and the surrounding metro, and it is the most specific published offer on this list.

What it is genuinely good at. It publishes what the others describe vaguely. Builds start at $5,500 one time, with a four payment option of a $2,000 deposit plus four payments of $875, and optional maintenance from $150 to $500 monthly, no contracts. Where to look closely. Owner operated cuts both ways. The person selling is the person building and answering, which is a real advantage on accountability, and it is a real risk on capacity and continuity. Ask what happens if the site breaks while he is on vacation, and ask how many active clients he supports. A one time build also means the site is yours to maintain after launch unless you add the maintenance plan, so decide up front which model you actually want.

5. Backyard Studios

What it is genuinely good at. Where to look closely. The published testimonials are from clients spread across Florida, Michigan, Arizona, Ohio and Tennessee rather than Texas, so this is a national practice with an Austin address rather than a Texas focused one. Confirm that the HIPAA posture extends to a BAA with you specifically and covers the photo library, not only internal training and vendor agreements. Pricing is not published.

Best fit. A clinic that wants a healthcare specialist agency and values a stated compliance posture over local proximity.

7. Envisager Studio

Envisager Studio is a design and marketing studio based in San Diego, California, with San Antonio aesthetic work in its portfolio through Magnolia Medical and Aesthetics.

What it is genuinely good at. PilotPractice markets itself as a San Antonio med spa digital marketing agency and states it was named a top med spa digital marketing agency in San Antonio by Clutch for 2024 and 2025, with a 5.0 Clutch rating and a case study reporting a 200 percent booking increase for CNT Beauty MedSpa.

What it is genuinely good at. Breadth of healthcare coverage, spanning dentistry, dermatology, plastic surgery, mental health, cardiology, orthopedics and more, with a full stack of services including SEO, paid search, social, email and SMS, web design, reputation management, CRM and scheduling. A clinic that wants marketing systems and the website from one vendor will find the offer complete.

Where to look closely, and this is a factual observation rather than a judgment. The San Antonio address it publishes is 700 North Saint Mary's Street, and the phone number it publishes carries a 480 area code, which is metropolitan Phoenix in Arizona. We noted the same pattern for this company during research for a previous list in a different vertical. A local address paired with an out of state phone number usually indicates a coworking or virtual office rather than a staffed operation. That does not make the work bad and remote agencies deliver good sites constantly. It does mean you should not choose this vendor because you believe the team is across town, so ask plainly where the people who will do your work actually sit.

One more thing to weigh. The headline offer is fifteen clients a month or you do not pay. Read the terms of any guarantee like that carefully, particularly what counts as a client, what the clinic is required to do for the guarantee to hold, and what the remedy actually is.

Best fit. A clinic that wants a broad healthcare marketing stack from one vendor and is comfortable with a remote team.
